Child and Young Person’s Programs
Child/Young Person’s Disability Program
The Child and Young Person’s Program provides regular support to families of children and young people aged 0 – 20 yrs with a moderate, severe or profound disability through respite and social support.
The AFFIRM Program provides intensive, flexible support to families of children and young people aged 7 – 20 yrs with a disability, complex needs and where the family is at risk of breakdown.
Adult Programs
The Adult Disability Program provides regular and ongoing support to adults living with a moderate, severe or profound disability through assistance with activities of daily living, respite and social support.
The Adult Mental Health Program provides regular support to adults living with a mental illlness through assistance with activities of daily living, respite and social support.
The Frail Aged Program provides regular and ongoing support to people who are over 65 years of age and frail aged through assistance with activities of daily living, respite and social support.
The LeisureLink Volunteer Program provides flexible support and friendship to people with a disability, people with mental illness and frail aged people.
After Hours On Call
Our After Hours Emergency On Call system is for:
- Tandem clients and support workers who need to notify Tandem of an incident, accident, sudden illness or other occurrence which has resulted in an urgent need to notify the office of a change in circumstances or support,
- and which is not able to wait until the office is next open
