SUPPORTING CHILDREN AND YOUNG PEOPLE WITH DISABILITIES

This program offers ongoing and regular support to support and strengthen families of children and young people with a moderate, severe or profound disability, or complex medical needs.  

A flexible and individualised model of support is provided to encourage positive and age appropriate outcomes for children and young people to achieve short and long term goals, as well as providing the much needed respite required for families. 

Options available include :

Home Based Support
Support based in the individual or family’s home. This may include the Support Worker providing support while there is a carer present or having sole responsibility for the child or young person, and may include responsibility for siblings.
  
Recreational Based Support
A Support Worker taking the child or young person out into the community to participate in social opportunities and/or develop social skills.
 
Respite Program
This program offers support as a period of relief, respite or support, for the individual or family. Respite may include the provision of personal care and assistance with daily life tasks such as grooming, showering, dressing and feeding, supervision of recreational and developmental needs, and general household duties.

Eligibility

Children and/or young people between 0 and 20 years of age who have a moderate, severe or profound disability of an intellectual, physical, sensory, medical or behavioural nature and live in the ACT.
